Schedule:

Responsibilities:
- Conduct comprehensive annual and post-discharge home health risk assessments for Medicare Advantage plan members
- Assess prior and current medical history, medication usage, and chronic conditions
- Educate patients regarding diagnosed conditions and medication management
- Identify gaps in care and recommend solutions to improve patient outcomes
- No requirements for treatment plan creation or medication prescribing
Qualifications:
- Active Nurse Practitioner License in Ohio
- Board Certification through AANP or ANCC
- Current CPR certification (BLS)
- Minimum of 6 months experience as a Nurse Practitioner is preferred
- MSN from an accredited nursing program
Each home health risk assessment typically takes about 45 minutes, and assignments may involve travel within Sidney, Fort Loramie, Anna, and nearby Shelby County communities. In addition to pay per completed evaluation, you will receive a mileage stipend and compensation for administrative time.
