Job Description

This per diem position allows you to provide valuable health insights to Medicare Advantage plan members in the comfort of their homes, with the flexibility to tailor your own schedule.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Create your own work schedule
  • Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

  • Conduct comprehensive annual home health risk assessments for Medicare Advantage members
  • Review and evaluate prior and current medical history, medical conditions, and medication use
  • Educate patients on medical conditions and medication management
  • Identify gaps in care and recommend actionable solutions
  • Visits do not involve creating treatment plans or prescribing medications

Qualifications:

  • Nurse Practitioner license valid in Ohio
  • Active CPR certification (BLS)
  • Board certification via AANP or ANCC
  • At least 6 months of recent experience as an NP is preferred
  • Master of Science in Nursing (MSN) from an accredited program
