Job Description

Deliver comprehensive care as a Nurse Practitioner specializing in home health risk assessments for Medicare Advantage plan members throughout Miami County. This per diem position offers the opportunity to make a direct impact on patient well-being while enjoying outstanding flexibility. As part of this role, you will be scheduled in locations including Troy, Piqua, and Tipp City, with each assessment taking about 45 minutes and performed in members homes. Evaluations may include annual health assessments or post-discharge visits.

Schedule/Hours:

  • Flexible shifts available Monday through Sunday
  • Self-scheduledcreate shifts that suit your lifestyle
  • Minimum weekly or bi-weekly hourly commitments required

Key Responsibilities:

  • Conduct thorough annual home health risk assessments for Medicare Advantage members
  • Assess patients prior and current medical history, current conditions, and medication lists
  • Provide clear education on disease processes and medication management
  • Identify care gaps and offer actionable recommendations to address them
  • No need to create treatment plans or prescribe medications

Qualifications:

  • Active Nurse Practitioner license issued in Ohio
  • Board Certification through AANP or ANCC
  • Current CPR (BLS) certification required
  • Master of Science in Nursing (MSN) degree from an accredited program
  • At least six months of recent NP experience preferred
