Job Description

Seeking a skilled Nurse Practitioner to perform home health risk assessments within Adams County, OH. This per diem role offers flexible scheduling, enabling you to create your own work hours between Monday and Sunday with a minimum weekly or bi-weekly hourly commitment. Evaluate and educate Medicare Advantage plan members during visits that typically last about 45 minutes.

Key Responsibilities:

  • Conduct comprehensive annual home health risk assessments for Medicare Advantage members.
  • Review prior and current medical history, medical conditions, and medication history.
  • Provide clear education regarding medical conditions and medications.
  • Identify gaps in care and recommend appropriate solutions.
  • Visit patients across Adams County locations including Peebles, West Union, and Manchester.
  • Note: This role does not involve creating treatment plans or prescribing medications.

Qualifications:

  • Active Nurse Practitioner License in Ohio.
  • Current CPR certification (BLS).
  • Board Certification through AANP or ANCC.
  • Minimum of 6 months current NP experience preferred.
  • Master of Science in Nursing (MSN) from an accredited school.

Schedule:

  • Flexible shifts Monday through Sunday.
  • Create your own schedule within a minimum weekly or bi-weekly commitment.
