Job Description

A Nurse Practitioner is needed to conduct comprehensive home health risk assessments across Highland County, Ohio. This per diem position offers flexible scheduling with opportunities to work in various locations including Hillsboro, Greenfield, and Leesburg. Candidates will evaluate Medicare Advantage plan members' health status during visits that generally take about 45 minutes, ranging from annual assessments to post-discharge follow-ups.

Key Responsibilities:

  • Perform detailed annual home health risk assessments for Medicare Advantage members
  • Review patients prior and current medical histories, conditions, and medication use
  • Educate patients on their medical conditions and prescribed medications
  • Identify gaps in care and provide actionable recommendations for improvement
  • No responsibilities involving treatment plan creation or medication prescribing

Qualifications:

  • Active Nurse Practitioner license in Ohio
  • Current Basic Life Support (BLS) certification
  • Board certification from AANP or ANCC
  • Preferred minimum of 6 months practical experience as an NP
  • Master of Science in Nursing (MSN) from an accredited institution

Schedule:

  • Flexible shifts available Monday through Sunday
  • Ability to create your own schedule
  • Minimum commitment of weekly or bi-weekly hours
