Job Description

Provide comprehensive home health risk assessments for Medicare Advantage plan members in Adams County, OH, including Peebles, West Union, and Manchester. This per diem role offers flexible scheduling between Monday and Sunday, allowing you to create a work schedule that fits your lifestyle, with a minimum hourly commitment weekly or bi-weekly.

Key Responsibilities:

  • Conduct thorough annual home health risk assessments, averaging 45 minutes per visit
  • Evaluate patients medical history, current medical conditions, and medication usage
  • Educate patients on their medical conditions and prescribed medications
  • Identify gaps in care and offer recommendations to improve patient outcomes
  • Visits may also include post-discharge assessments
  • No requirement to create treatment plans or prescribe medications

Qualifications:

  • Active Nurse Practitioner license in Ohio
  • Current BLS/CPR certification
  • Board Certification from AANP or ANCC
  • Preferred minimum of six months recent NP experience
  • MSN degree from an accredited nursing program
