Job Description

Deliver quality patient care from the comfort of patients homes as a nurse practitioner specializing in home health risk assessments. This per diem role focuses on delivering comprehensive evaluations to Medicare Advantage plan members residing throughout Preble County, including the communities of Eaton, Camden, and Lewisburg. Each visit lasts approximately 45 minutes and may serve as an annual health assessment or a post-discharge evaluation.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Opportunity to create your own schedule
  • Commitment can be tailored weekly or bi-weekly to fit your availability

Key Responsibilities:

  • Conduct in-home risk assessments for Medicare Advantage members
  • Review and evaluate medical history, current health conditions, and medication usage
  • Educate patients on their diagnoses, ongoing care, and medication regimens
  • Identify care gaps and make sound recommendations to address outstanding health needs
  • Note: This position does not require prescribing medications or formulating treatment plans

Required Qualifications:

  • Active Nurse Practitioner license in Ohio
  • Board Certification through AANP or ANCC
  • Current CPR (BLS) certification
  • MSN degree from an accredited institution
  • Six months recent experience as an NP is preferred
