Job Description

This role focuses on providing essential risk assessments for Medicare Advantage members in diverse communities across Preble County. With flexible scheduling and the chance to work independently, youll play a vital role in enhancing healthcare outcomes for individuals residing in settings such as Eaton, Camden, and Lewisburg.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Set your own schedule based on availability
  • Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

  • Complete thorough annual and post-discharge health risk assessments in members homes
  • Review and evaluate both prior and current medical history, conditions, and medications
  • Educate patients on their health conditions and prescribed medications
  • Identify gaps in care and offer detailed recommendations for improvement
  • No requirement to create treatment plans or prescribe medications

Desired Qualifications:

  • Active Nurse Practitioner license in Ohio
  • Board Certification through AANP or ANCC
  • Current BLS (CPR) certification
  • MSN degree from an accredited nursing school
  • At least 6 months of recent Nurse Practitioner experience preferred

This role offers competitive compensation of $80 to $90 per evaluation, with an additional mileage stipend and pay for administrative time.
