Step into a dynamic per diem opportunity as a Nurse Practitioner delivering home health risk assessments for Medicare Advantage members in Scioto County, OH. This flexible role allows you to make a lasting impact on patient care with a schedule that fits your lifestyle.

Schedule:

• Flexible shifts available Monday through Sunday
• Set your own schedule
• Commit to a minimum number of hours weekly or every other week

Key Responsibilities:

• Perform in-home annual health risk assessments for Medicare Advantage plan members within Scioto County, including Portsmouth, Porter Township, and Clay Township
• Assess medical history, current medical conditions, and medications during each visiteach assessment typically lasting about 45 minutes
• Educate members on their health conditions and prescribed medications
• Identify gaps in care, offering actionable recommendations to support improved patient outcomes
• Visits may include annual comprehensive assessments or post-discharge evaluations
• No prescribing or creation of treatment plans required

Qualifications:

• Active Nurse Practitioner License in Ohio
• Current BLS/CPR certification
• Board Certification through AANP or ANCC
• Master of Science in Nursing (MSN) from an accredited program
• At least 6 months of recent Nurse Practitioner experience preferred
