Bring your clinical expertise to the forefront as a Nurse Practitioner specializing in home health risk assessments. In this per diem role, youll support Medicare Advantage plan members in various areas throughout Scioto County, including Portsmouth, Porter Township, and Clay Township. Schedule:
- Flexible shifts available Monday through Sunday
- Create your own work schedule
- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Conduct annual and post-discharge home health risk assessments for Medicare Advantage members
- Review and evaluate prior and current medical history, as well as medication history
- Deliver patient education on specific medical conditions and prescription use
- Identify gaps in care and supply actionable recommendations
- No requirement for developing treatment plans or issuing prescriptions
Qualifications:
- Active Nurse Practitioner license in the state of Ohio
- MSN degree from an accredited nursing program
- Board certification from AANP or ANCC
- Current BLS/CPR certification
- At least 6 months of recent Nurse Practitioner experience preferred
