This role offers the opportunity to provide essential health assessments for Medicare Advantage members within a variety of communities across Huron County, OH. Working as a Nurse Practitioner focused on home health, you will conduct thorough risk assessments that support patient well-being without the responsibility of creating treatment plans or prescribing medications. The position is per diem with highly flexible scheduling options, ideal for professionals seeking autonomy in their work hours.

Location:
Huron County, OH, encompasses charming communities including Norwalk, Willard, and Bellevue. This region combines a welcoming small-town atmosphere with access to local amenities and natural beauty. Serving diverse populations across these areas, you will contribute to improving healthcare outcomes and assisting patients in maintaining healthier lives in their homes.

Responsibilities:

  • Conduct comprehensive annual home health risk assessments for Medicare Advantage plan members
  • Review and evaluate prior and current medical history, conditions, and medications
  • Educate patients on their medical conditions and medication management
  • Identify gaps in care and make recommendations to support resolution
  • Travel may be required within Norwalk and neighboring communities in Huron County

Qualifications and Desired Experience:

  • Active Nurse Practitioner license in Ohio
  • Board Certification through AANP or ANCC
  • MSN degree from an accredited nursing program
  • Current CPR certification (BLS)
  • A minimum of 6 months current NP experience preferred

Schedule:

  • Flexible shifts available Monday through Sunday
  • Create your own schedule based on your availability
  • Minimum weekly or bi-weekly hourly commitment
