Job Description

Provide high-quality home health risk assessments as a Nurse Practitioner (NP) serving Medicare Advantage plan members across Erie County, OH. This per diem opportunity offers flexible scheduling and the ability to make a meaningful impact on patient outcomes through comprehensive evaluations and education in patients' homes.

Key Responsibilities:

  • Conduct annual home health risk assessments, including post-discharge visits, averaging 45 minutes per visit
  • Evaluate patients prior and current medical history, medical conditions, and medication usage
  • Educate patients on their medical conditions and prescribed medications
  • Identify gaps in care and recommend appropriate resolutions
  • No treatment plan creation or medication prescribing required

Qualifications:

  • Current Nurse Practitioner License in Ohio
  • Board Certification through AANP or ANCC
  • Minimum of 6 months recent NP experience preferred
  • Master of Science in Nursing (MSN) from an accredited program
  • Current CPR certification (BLS)

Schedule:

  • Flexible shifts available Monday through Sunday
  • Create your own schedule with a minimum weekly or bi-weekly commitment
  • Assignments may be anywhere within Erie County, including Sandusky, Huron, Vermilion, and Norwalk areas
