Job Description

Make a meaningful impact by providing in-home risk assessments for Medicare Advantage plan members across Stark County. This per diem opportunity offers Nurse Practitioners the flexibility to balance their clinical skills with a personalized schedule, while ensuring quality care and health education.

Schedule:

  • Flexible shifts available seven days a week
  • Self-scheduled hours for optimal work-life balance
  • Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

  • Perform comprehensive annual risk assessments in home settings, each lasting approximately 45 minutes
  • Review and evaluate medical history, chronic conditions, and medication use
  • Deliver health education tailored to each patients needs, focusing on medical conditions and medication adherence
  • Identify potential gaps in care, offering professional recommendations without developing treatment plans or prescribing medications
  • Complete assessments for individuals throughout Stark County, including areas such as Canton, Massillon, and Alliance; assignments may rotate per shift

Qualifications:

  • Licensed Nurse Practitioner in the state of Ohio
  • Board Certified through AANP or ANCC
  • Possess an MSN degree from an accredited institution
  • Current CPR certification (BLS)
  • At least six months of recent NP experience preferred
