Job Description

Advance your clinical career by providing comprehensive home health risk assessments to Medicare Advantage members throughout Stark County, Ohio. As a Nurse Practitioner, youll play a pivotal role in supporting individuals ongoing healthcare needs through thorough evaluations in their homes. This per diem opportunity features flexibility and meaningful patient engagementideal for experienced NPs passionate about improving patient outcomes outside a traditional clinical setting.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Create your own schedulework as often as youd like
  • Minimum weekly or bi-weekly hourly commitments to fit your availability

Key Responsibilities:

  • Conduct annual health risk assessments for Medicare Advantage plan members in their homes
  • Evaluate prior and current medical history, assess current medical conditions, and review medication history
  • Provide education to patients on health conditions and appropriate medication usage
  • Identify care gaps and offer clinical recommendations to help optimize patient care
  • Visits average approximately 45 minutes each; types of visits include annual assessments or post-discharge evaluations
  • No requirement to develop treatment plans or prescribe medications

Qualifications:

  • Active Nurse Practitioner license in Ohio
  • Current certification in CPR (BLS)
  • Board Certified by AANP or ANCC
  • MSN degree from an accredited nursing program
  • Minimum of 6 months current NP experience preferred
