Make a meaningful impact by providing in-home risk assessments for Medicare Advantage plan members across Stark County. This per diem opportunity offers Nurse Practitioners the flexibility to balance their clinical skills with a personalized schedule, while ensuring quality care and health education.
Schedule:
- Flexible shifts available seven days a week
- Self-scheduled hours for optimal work-life balance
- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Perform comprehensive annual risk assessments in home settings, each lasting approximately 45 minutes
- Review and evaluate medical history, chronic conditions, and medication use
- Deliver health education tailored to each patients needs, focusing on medical conditions and medication adherence
- Identify potential gaps in care, offering professional recommendations without developing treatment plans or prescribing medications
- Complete assessments for individuals throughout Stark County, including areas such as Canton, Massillon, and Alliance; assignments may rotate per shift
Qualifications:
- Licensed Nurse Practitioner in the state of Ohio
- Board Certified through AANP or ANCC
- Possess an MSN degree from an accredited institution
- Current CPR certification (BLS)
- At least six months of recent NP experience preferred
