Provide comprehensive home health risk assessments as a Nurse Practitioner serving Medicare Advantage plan members throughout Guernsey County, OH. This per diem opportunity offers the flexibility to create your own schedule with shifts available Monday through Sunday, making it ideal for professionals seeking variable hours and meaningful patient interactions.
Responsibilities include conducting thorough annual health risk assessments that typically last about 45 minutes. You will evaluate patients medical and medication histories, deliver education on health conditions and treatments, identify gaps in care, and propose actionable recommendations. This role focuses on assessment and education without the need for treatment plan creation or medication prescribing.
Key Responsibilities:
- Perform annual home health risk assessments for Medicare Advantage beneficiaries
- Review prior and current medical and medication histories
- Educate patients on managing their medical conditions and medications
- Identify gaps in care and recommend appropriate interventions
- Complete documentation accurately in a timely manner
Qualifications:
- Active Nurse Practitioner license in Ohio
- Current BLS (CPR) certification
- Board Certified through AANP or ANCC
- Minimum six months of NP experience preferred
- Master of Science in Nursing (MSN) from an accredited institution
