Take your advanced practice skills to the next level by supporting patients residing in skilled nursing and assisted living settings. In this contract opportunity, youll play a crucial role in providing comprehensive care for insurance members, with a strong focus on in-place treatment to reduce avoidable hospital admissions and ensure smooth transitions home. The emphasis is on high-quality, ongoing patient assessments and resource coordination to improve health outcomes within the comfort of long-term care facilities.
Schedule:
- Monday through Friday, 8 AM to 5 PM
- No weekends or holidays
Key Responsibilities:
- Perform thorough health assessments, with a minimum of monthly visits for each resident and additional assessments as clinically indicated
- Develop individualized problem lists and treatment plans tailored to each patients needs
- Deliver primary, preventive, urgent, and acute care directly within designated facilities
- Stratify patients by risk, scheduling more frequent visits for those at higher acuity, and establishing proactive contingency plans
- Collaborate closely with each residents primary care physician, facility staff, Director of Nursing, and multidisciplinary teams to ensure holistic care
- Manage hands-on patient care, create and update care plans and orders, and help orchestrate safe transitions for patients newly discharged from hospitals to skilled nursing facilities
- Travel between multiple assigned facilities as needed, with scheduling support to maximize patient continuity
Qualifications:
- Master of Science in Nursing (MSN degree) required
- Active Nurse Practitioner license in New York State
- Current Basic Life Support (BLS) certification
- Background in primary care, geriatrics, or long-term care environments is highly desirable
