Step into a dynamic opportunity leveraging your skills as a Nurse Practitioner to make a meaningful impact in skilled nursing and assisted living settings. This contract role centers on delivering comprehensive care for insurance members residing in nursing homes and assisted living facilities. Efforts are focused on treating patients in place when possible, reducing unnecessary hospitalizations, and ensuring smooth transitions back to the home setting.

Schedule:

• Monday through Friday, 8:00 AM to 5:00 PM
• No weekend or holiday shifts required

Key Responsibilities:

• Conduct monthly (or more frequent as needed) health assessments for each resident
• Develop problem lists and medical treatment plans tailored to individual patient needs
• Deliver primary, preventive, urgent, and acute care services on-site
• Identify and stratify high-risk patients, providing more frequent visits and creating contingency plans as needed
• Collaborate and communicate with residents primary care physicians and interdisciplinary teams, including Directors of Nursing and facility staff
• Provide hands-on bedside care, develop care plans, and issue necessary orders within assigned facilities
• Oversee patients transitioning into facilities post-hospital discharge, ensuring safe and effective transfer out of skilled nursing when appropriate
• Manage a panel of members across multiple facilities, typically scheduled at one primary location per day, with flexibility to respond to acute needs at other sites as required

Qualifications:

• Master of Science in Nursing (MSN)
• Current BLS certification
• Active Nurse Practitioner license for New York

Earn between $75 and $100 per hour while contributing your expertise to improve patient outcomes and overall healthcare delivery in post-acute settings.
