Key Responsibilities:

  • Complete comprehensive home health risk assessments for Medicare Advantage members
  • Assess medical history, current conditions, and medication use
  • Offer education to patients on chronic health issues and medication management
  • Identify care gaps and recommend follow-up actions to promote optimal patient health
  • Visits do not require developing treatment plans or prescription management

Qualifications:

  • Active Nurse Practitioner License in OH
  • Board Certification via AANP or ANCC
  • Current BLS (CPR) Certification
  • MSN degree from an accredited nursing program
  • At least six months of NP experience (preferred)

This role offers competitive compensation at $80-$90 per completed evaluation, with additional mileage reimbursement and payment for administrative time.
