Embark on a rewarding per diem opportunity as a Nurse Practitioner delivering home health risk assessments throughout Fayette County, OH, including Washington Court House, Jeffersonville, and Bloomingburg. Schedule:

Key Responsibilities:
- Complete comprehensive home health risk assessments for Medicare Advantage members
- Assess medical history, current conditions, and medication use
- Offer education to patients on chronic health issues and medication management
- Identify care gaps and recommend follow-up actions to promote optimal patient health
- Visits do not require developing treatment plans or prescription management
Qualifications:
- Active Nurse Practitioner License in OH
- Board Certification via AANP or ANCC
- Current BLS (CPR) Certification
- MSN degree from an accredited nursing program
- At least six months of NP experience (preferred)
This role offers competitive compensation at $80-$90 per completed evaluation, with additional mileage reimbursement and payment for administrative time.
