Embark on a rewarding opportunity as a Nurse Practitioner delivering personalized care through home health risk assessments. This per diem role prioritizes flexibility and autonomy, enabling providers to create their own schedules while supporting Medicare Advantage members in the greater Portage County area, with visits ranging from annual assessments to post-discharge check-ins. Each home visit typically lasts approximately 45 minutes, serving communities such as Kent, Ravenna, and Streetsboro.

What Youll Do
- Conduct comprehensive in-home risk assessments for Medicare Advantage plan members
- Review prior and current medical history, conditions, and medications
- Deliver education to members concerning health conditions and prescribed treatments
- Identify care gaps, offering actionable recommendations for improvement
- Documentation-focused role: no prescribing or treatment planning necessary
Desired Qualifications
- Active Nurse Practitioner (NP) license in Ohio
- Board certification from AANP or ANCC
- Current CPR (BLS) certification
- MSN degree from an accredited nursing program
- At least 6 months of Nurse Practitioner experience strongly preferred
