Job Description
Deliver essential home-based healthcare by conducting comprehensive risk assessments as a Nurse Practitioner serving Medicare Advantage plan members throughout Meigs County. This per diem opportunity allows the flexibility to choose your own schedule and make a direct impact on patient well-being across various communities in the county.
Schedule:
- Flexible shifts available Monday through Sunday
- Create your own schedule to fit your lifestyle
- Minimum weekly or bi-weekly hourly commitment required
Responsibilities:
- Perform thorough annual health assessments for Medicare Advantage members in Meigs County, including Salisbury Township, Pomeroy, Rutland Township, and Chester Township
- Evaluate prior and current medical histories, including any chronic or acute conditions, and conduct medication reviews
- Educate members on their diagnoses, treatment options, and medication regimens
- Identify gaps in care and offer practical recommendations to resolve issues
- Visits typically last about 45 minutes and may also include post-discharge follow-ups as needed
- No prescribing or treatment plan development necessaryfocus on assessment, education, and care coordination
Qualifications:
- Active Nurse Practitioner license in the state of Ohio
- Current CPR certification (BLS)
- National board certification through AANP or ANCC
- MSN degree from an accredited nursing school
- At least 6 months of recent NP experience preferred
You will receive compensation of $80$90 per evaluation plus a mileage stipend and reimbursement for administrative time.
