Job Description

Deliver impactful care where it matters most by joining an experienced healthcare team conducting home health risk assessments for Medicare Advantage plan members. This per diem opportunity empowers Nurse Practitioners to make a real difference across Summit County, including Akron, Cuyahoga Falls, Stow, and neighboring communities. Each assessment takes approximately 45 minutes, ranging from annual evaluations to post-discharge visits, and ensures better health outcomes for vulnerable populationsall while offering flexibility and autonomy for providers.

  • Flexible shifts available Monday through Sunday
  • Create your own schedule to match your availability
  • Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities

  • Conduct comprehensive annual home health risk assessments for Medicare Advantage members
  • Review and evaluate medical history, prior and present conditions, and current medication regimens
  • Educate clients on their health conditions and prescribed medications
  • Identify gaps in care and advise on appropriate next steps for resolution
  • No requirements for creating treatment plans or writing prescriptions

Qualifications

  • Active Nurse Practitioner License in Ohio
  • Board Certification via AANP or ANCC
  • Masters degree in Nursing (MSN) from an accredited institution
  • Current BLS certification (CPR required)
  • At least 6 months of current NP experience preferred
