Locations may include Cuyahoga Falls, Stow, Gree, and Akron, with each assignment presenting a unique opportunity to make an impact in the community.

Schedule:

  • Flexible shifts available any day of the week
  • Self-scheduling with minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Perform comprehensive annual risk assessments for homebound Medicare Advantage patients
  • Gather detailed medical histories, assess current health conditions, and review medications
  • Educate patients on their diagnoses, medications, and general wellness practices
  • Identify any gaps in care and recommend follow-up actions as needed
  • Visits require no treatment plan creation or prescription writingfocus is on assessment, documentation, and support

Qualifications:

  • Active Nurse Practitioner (NP) License in Ohio
  • Current CPR certification (BLS)
  • Board certified through either AANP or ANCC
  • Minimum of 6 months experience as a practicing NP preferred
  • MSN degree from an accredited nursing program
