Leverage your expertise and clinical skills in a dynamic per diem role as a Nurse Practitioner, providing impactful home health risk assessments throughout Stark County. This opportunity offers the autonomy to create your own schedule and to make a difference in the lives of individuals managing chronic health conditions in their homes.
Schedule/Hours:
- Flexible shifts available, Monday through Sunday

- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Perform comprehensive annual home health risk assessments for Medicare Advantage plan members
- Evaluate past and current medical history, conditions, and medication usage
- Deliver clear education to individuals regarding health status, medical conditions, and prescribed medications
- Identify care gaps and recommend solutions without being responsible for creating treatment plans or prescribing medications
- Complete each home visit, which averages 45 minutes, in a professional and compassionate manner
Qualifications:
- Current Nurse Practitioner License in Ohio
- CPR certification (BLS) required
- Board certification through AANP or ANCC
- Minimum of six months of current Nurse Practitioner experience preferred
- MSN from an accredited School of Nursing
This per diem role offers a competitive earnings structure$80-$90 per evaluationplus a mileage stipend and compensation for administrative time.
