Take on a rewarding per diem opportunity as a Nurse Practitioner specializing in home health assessments. In this flexible role, youll have the autonomy to create your own schedule, offering services to Medicare Advantage plan members in the Portage County area, including Kent, Ravenna, and Streetsboro. Each visit, which averages about 45 minutes, may encompass annual assessments or post-discharge follow-ups to ensure patients receive comprehensive care and support in their homes.
Schedule:
- Flexible shifts available any day, Monday through Sunday

- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Complete in-home health risk assessments for Medicare Advantage members
- Thoroughly evaluate patients medical histories, current conditions, and medication use
- Offer education and counseling on chronic medical conditions and medication regimens
- Identify gaps in care and provide recommendations to address them
- Note: No prescription writing or treatment plan development is required
Qualifications:
- Valid Nurse Practitioner license in Ohio
- Board certification through AANP or ANCC
- Current CPR/BLS certification
- Master of Science in Nursing (MSN) from an accredited nursing program
- At least 6 months of recent Nurse Practitioner experience preferred
