Experienced Nurse Practitioner needed for per diem home health assessments in Mason, OH and surrounding Hamilton County areas. This role focuses on conducting comprehensive home health risk assessments for Medicare Advantage plan members, providing valuable insights to support patient well-being.
Key Responsibilities:
- Perform annual home health risk assessments, including post-discharge visits within Hamilton County, OH
- Review and evaluate patients medical history and medication use
- Educate patients on their medical conditions and prescribed medications
- Identify gaps in care and recommend solutions to enhance patient outcomes
- Documentation and communication without treatment plan creation or prescribing
Qualifications:
- Active Nurse Practitioner license in Ohio
- Current Basic Life Support (BLS) certification
- Board certification through AANP or ANCC
- Minimum of 6 months of recent NP experience preferred
- Master of Science in Nursing (MSN) from an accredited institution
Schedule:
- Flexible shifts available Monday through Sunday
- Create your own schedule based on minimum weekly or bi-weekly hour commitments
- Per diem employment with the ability to balance work and personal life
