As a Nurse Practitioner, youll provide valuable insights to improve patient outcomes in the comfort of their homes across areas like Martins Ferry, Richland Township, Pease Township, and Pultney Township. Schedule:

• Flexible shifts available Monday through Sunday
• Self-scheduling for optimal work/life balance
• Choose your minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

• Perform detailed annual home health risk assessments for Medicare Advantage plan members
• Review and evaluate patients prior/current medical conditions, history, and medications
• Educate patients about their health conditions and medications
• Identify gaps in care and offer actionable recommendations
• No requirement to create treatment plans or prescribe medications

Qualifications:

• Active Nurse Practitioner license in Ohio
• Current CPR certification (BLS)
• Board Certification through AANP or ANCC
• Minimum of six months NP experience preferred
• MSN degree from an accredited nursing school
