Job Description

Providing comprehensive home health risk assessments for Medicare Advantage members in Ashtabula County, OH, this per diem Nurse Practitioner role offers flexible scheduling and meaningful work in the community. You will conduct approximately 45-minute visits to evaluate health status, review medical history, and identify care gapsall without the responsibilities of prescribing or creating treatment plans.

Key Responsibilities

  • Perform annual home health risk assessments for Medicare Advantage plan members
  • Review prior and current medical history, medical conditions, and medication history
  • Educate patients on their medical conditions and medications
  • Identify gaps in care and recommend solutions to enhance patient outcomes
  • Conduct visits within Ashtabula County locations such as Ashtabula, Conneaut, and Geneva

Schedule

  • Flexible shifts available Monday through Sunday
  • Create your own schedule with a minimum weekly or bi-weekly hourly commitment
  • Per diem work allows for balance and autonomy in choosing when to provide care

Qualifications

  • Active Nurse Practitioner license in Ohio
  • Current CPR (BLS) certification
  • Board Certification through AANP or ANCC
  • MSN degree from an accredited school of nursing program
  • Minimum of 6 months current NP experience preferred
