Discover a rewarding per diem opportunity as a Nurse Practitioner specializing in home health risk assessments throughout Scioto County. This role is focused on providing quality care to Medicare Advantage plan members during in-home visits. With flexible shifts available between Monday and Sunday and the ability to create your own schedule, youll have the freedom to balance your work and personal life while making a direct impact in the communities of Portsmouth, Porter Township, and Clay Township.
Schedule:
- Flexible shifts offered seven days a week
- Craft your own work schedule
- Minimum weekly or bi-weekly hourly commitment, as preferred
Key Responsibilities:
- Conduct comprehensive annual health risk assessments for Medicare Advantage members in their homes
- Evaluate prior and current medical history, including chronic conditions and medication profiles
- Educate patients about their health conditions and medications
- Identify and document gaps in care, offering recommendations for follow-up or resolution
- Focus exclusively on assessmentsno treatment planning or prescribing medications involved
Qualifications:
- Active Nurse Practitioner License in Ohio
- Current BLS/CPR certification
- Board certified through AANP or ANCC
- Preferred: At least 6 months of recent Nurse Practitioner experience
- MSN earned from an accredited nursing program
