Job Description

A Nurse Practitioner role is available to perform home health risk assessments within Hamilton County, Ohio. This per diem position offers flexible scheduling with shifts available Monday through Sunday, allowing you to create your own schedule with a minimum weekly or bi-weekly hour commitment.

Responsibilities include conducting annual health risk assessments for Medicare Advantage plan members residing in Cincinnati, Forest Park, Norwood, and surrounding Hamilton County areas. Each assessment typically requires about 45 minutes and may involve reviewing medical history, current conditions, and medication usage. You will provide patient education, identify gaps in care, and recommend resolutions. This role does not require creating treatment plans or prescribing medications.

Key Responsibilities:

  • Conduct annual home health risk assessments for Medicare Advantage members
  • Review prior and current medical history, medical conditions, and medications
  • Educate patients on their medical conditions and medications
  • Identify care gaps and provide recommendations
  • Collaborate effectively within home health settings without prescribing duties

Qualifications:

  • Nurse Practitioner license in Ohio
  • Current CPR certification (BLS)
  • Board Certification through AANP or ANCC
  • At least 6 months of current NP experience preferred
  • MSN degree from an accredited school of nursing
