Job Description
Advance your nurse practitioner career with flexible per diem work focusing on home health risk assessments. In this role, youll visit Medicare Advantage plan members throughout Summit County, including Cuyahoga Falls, Stow, Gree, and Akron, providing vital annual assessments and helping close care gaps for patients in a home environment.
Schedule:
- Flexible shifts available Monday through Sunday
- Design your own schedule
- Minimum hourly commitment required weekly or bi-weekly
Key Responsibilities:
- Conduct comprehensive annual home health risk assessments for Medicare Advantage members
- Review and evaluate medical history, conditions, and current medication regimens
- Educate patients about their health conditions and treatments
- Identify care gaps and make recommendations for improved care
- Note: This position does not involve developing treatment plans or prescribing medications
Qualifications:
- Active Nurse Practitioner license in Ohio
- Board certification through AANP or ANCC
- Current CPR certification (BLS required)
- MSN degree from an accredited nursing program
- At least 6 months of current NP experience is preferred
