Job Description

A Nurse Practitioner is needed to conduct thorough home health risk assessments for Medicare Advantage plan members residing in Ashtabula County, OH. This per diem opportunity offers exceptional flexibility, allowing you to create your own schedule with shifts available any day from Monday through Sunday. Engage meaningfully with patients in their homes to evaluate health risks, provide education, and identify gaps in care.

Location
The role serves Ashtabula County, OH, including the cities of Ashtabula, Conneaut, and Geneva. This scenic region features beautiful lakefront views along Lake Erie, a mix of urban and rural communities, and a welcoming atmosphere, providing a pleasant setting for delivering compassionate care.

Responsibilities

  • Conduct annual home health risk assessments averaging 45 minutes per visit
  • Evaluate patients prior and current medical history and medication regimens
  • Educate patients on their medical conditions and prescribed medications
  • Identify gaps in care and make recommendations for improvement
  • No treatment plan creation or medication prescribing required

Qualifications

  • Valid Nurse Practitioner license in Ohio
  • Current BLS (CPR) certification
  • Board Certification through AANP or ANCC
  • Minimum six months of NP experience preferred
  • MSN degree from an accredited nursing program

Schedule

  • Flexible shifts available Monday through Sunday

  • Minimum weekly or bi-weekly hourly commitment
