Schedule:
- Flexible shifts available Monday through Sunday
- Choose your own schedule
- Commitment options: set your hours weekly or bi-weekly
Key Responsibilities:
- Perform comprehensive annual risk assessments during in-home visits (approximately 45 minutes each) for Medicare Advantage plan members
- Review and evaluate patients medical history, current conditions, and medications
- Deliver clear education about diagnoses, disease management, and medications during each assessment
- Identify and address gaps in care, providing actionable recommendations for follow-up
- Focus solely on assessmentsno requirements for treatment plans or prescribing
Qualifications:
- Current Nurse Practitioner (NP) license in the state of Ohio
- Valid Basic Life Support (BLS/CPR) certification
- Board Certified via AANP or ANCC
- At least 6 months of current NP experience preferred
- MSN degree from an accredited nursing program
This engaging per diem position offers a competitive payment of $80-$90 per evaluation performed, with additional compensation for mileage and administrative responsibilities. Youll have the freedom to create a customized schedule and visit a variety of locations throughout Fayette County, including Washington Court House, Jeffersonville, and Bloomingburg.
