Seeking a skilled Nurse Practitioner to perform home health risk assessments throughout Lawrence County, including areas such as Ironton, South Point, and Proctorville. This per diem opportunity is ideal for clinicians passionate about delivering personalized care and education within patients homes.

Schedule:

• Flexible scheduling options available Monday through Sunday
• Self-select your own shifts
• Commitment options: minimum weekly or bi-weekly hours

Responsibilities:

• Complete thorough home health risk assessments for Medicare Advantage plan members
• Conduct evaluations covering medical history, current conditions, and medication use
• Educate patients on their health conditions, management strategies, and prescribed medications
• Identify care gaps and make recommendations to improve patient outcomes
• No requirement to write prescriptions or create treatment plans; the focus is assessment and education

Qualifications:

• Licensed as a Nurse Practitioner in Ohio
• Board Certification through AANP or ANCC
• Active CPR (BLS) certification
• At least 6 months of recent Nurse Practitioner experience preferred
• MSN from an accredited nursing program

Nurse Practitioners in this position are compensated at $80-$90 per evaluation, with a mileage stipend and administrative time included. Each assessment typically takes about 45 minutes, with the nature of visits tailored to the patients needs, such as annual health assessments or post-discharge follow-ups.
