A Nurse Practitioner (NP) opportunity is available to conduct home health risk assessments within Huron County, OH. This per diem role offers flexible scheduling with the ability to create your own shifts between Monday and Sunday, requiring a minimum weekly or bi-weekly hourly commitment. The focus will be on Medicare Advantage plan members residing in communities such as Norwalk, Willard, and Bellevue.

This position allows you to engage meaningfully with patients in their homes across multiple locations within the county.
Responsibilities
- Conduct comprehensive annual home health risk assessments, including post-discharge visits, averaging approximately 45 minutes per evaluation
- Review prior and current medical history, medical conditions, and medication use
- Educate patients on their medical conditions and medication management
- Identify gaps in care and offer recommendations to support improved health outcomes
- Collaborate with care teams while adhering to scope of practicethis role does not involve creating treatment plans or prescribing medications
Qualifications
- Active Nurse Practitioner License in Ohio
- Current Basic Life Support (BLS) CPR certification
- Board Certification through AANP or ANCC
- Preferred minimum of 6 months recent NP clinical experience
- Master of Science in Nursing (MSN) degree from an accredited institution
