Job Description

Key Responsibilities:

  • Conduct thorough annual home health risk assessments averaging 45 minutes per visit
  • Evaluate patients prior and current medical history, medical conditions, and medication usage
  • Provide education on medical conditions and prescribed medications to support patient understanding
  • Identify gaps in care and recommend practical resolutions to improve health outcomes
  • Note: No treatment plan creation or medication prescribing is required

Qualifications:

  • Active Nurse Practitioner license in Ohio
  • Board certification through AANP or ANCC
  • Current Basic Life Support (CPR/BLS) certification
  • MSN degree from an accredited school of nursing
  • Minimum 6 months of recent NP clinical experience preferred

Schedule:

  • Flexible shifts available Monday through Sunday
  • Create your own schedule with minimal weekly or bi-weekly time commitments
