Join a dynamic healthcare role focused on delivering comprehensive home health risk assessments to Medicare Advantage members within Highland County, Ohio. Key Responsibilities:
- Conduct thorough annual home health risk assessments averaging 45 minutes per visit
- Evaluate patients prior and current medical history, medical conditions, and medication usage
- Provide education on medical conditions and prescribed medications to support patient understanding
- Identify gaps in care and recommend practical resolutions to improve health outcomes
- Note: No treatment plan creation or medication prescribing is required
Qualifications:
- Active Nurse Practitioner license in Ohio
- Board certification through AANP or ANCC
- Current Basic Life Support (CPR/BLS) certification
- MSN degree from an accredited school of nursing
- Minimum 6 months of recent NP clinical experience preferred
Schedule:
- Flexible shifts available Monday through Sunday
- Create your own schedule with minimal weekly or bi-weekly time commitments
