Provide comprehensive home health risk assessments as a Nurse Practitioner serving Medicare Advantage members in Adams County, OH. This per diem role offers flexible scheduling and the opportunity to make a meaningful impact in patients lives through thorough evaluations and patient education.
Location Overview:
Adams County, located in scenic southern Ohio, includes charming communities such as Peebles, West Union, and Manchester. This region offers a blend of rural landscapes and small-town values, providing a welcoming environment to deliver quality health care in patients homes.
Responsibilities:
- Conduct annual home health risk assessments averaging 45 minutes per visit
- Evaluate patients medical and medication histories
- Educate patients on medical conditions and medication usage
- Identify gaps in care and recommend actions to improve patient outcomes
- Visits may include post-discharge assessments without creating treatment plans or prescribing medications
Qualifications:
- Licensed Nurse Practitioner in Ohio
- Current BLS/CPR certification
- Board certification through AANP or ANCC
- Minimum six months of NP experience preferred
- Master of Science in Nursing (MSN) from an accredited program
Schedule:
- Flexible shifts available any day Monday through Sunday
- Create your own schedule to meet your availability
- Minimal commitment of weekly or bi-weekly hours
