Job Description

Deliver impactful patient care while enjoying true flexibility as a Nurse Practitioner in a dynamic per diem home health setting. In this role, you'll conduct comprehensive risk assessments with Medicare Advantage plan members residing in various locations across Columbiana County. Each home visit typically lasts about 45 minutes, allowing you to focus on each patient's unique needs. The types of assessments may range from annual health check-ups to post-discharge visits, providing rewarding variety in your day.

Schedule:

  • Flexible shifts offered seven days a week
  • Customize your own schedule
  • Minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Conduct thorough annual home health risk assessments for Medicare Advantage members
  • Assess current and prior medical history, health conditions, and medication usage
  • Educate patients on health conditions and medication management
  • Identify unmet healthcare needs and recommend solutions to close gaps in care
  • No requirement for developing treatment plans or prescribing medications

Qualifications:

  • Active and unrestricted Nurse Practitioner License in Ohio
  • Current CPR/BLS certification
  • Board Certification through AANP or ANCC
  • At least 6 months of recent experience as a Nurse Practitioner preferred
  • Graduate degree (MSN) from an accredited nursing program
