Job Description

Provide comprehensive home health risk assessments as a Nurse Practitioner serving Medicare Advantage plan members in Columbiana County, OH. This per diem opportunity offers flexibility to craft your own schedule with shifts available Monday through Sunday, supporting a work-life balance tailored to your needs. You will conduct thorough evaluations of medical history, current conditions, and medications during visits lasting approximately 45 minutes. Visits could range from annual health assessments to post-discharge follow-ups, covering locations such as Bucyrus, Galion, Crestline, and surrounding areas.

Key Responsibilities:

  • Conduct annual home health risk assessments for Medicare Advantage members
  • Evaluate prior and current medical history, medical conditions, and medication history
  • Provide patient education on medical conditions and medications
  • Identify gaps in care and recommend appropriate resolutions
  • No treatment plan creation or medication prescribing required

Qualifications:

  • Valid Nurse Practitioner license in Ohio
  • Current CPR certification (BLS)
  • Board Certification through AANP or ANCC
  • Minimum of 6 months current NP experience preferred
  • MSN degree from an accredited school of nursing

Schedule:

  • Flexible shifts available Monday through Sunday
  • Create your own schedule with minimum weekly or bi-weekly commitment
