Exciting opportunities are available for experienced Nurse Practitioners seeking a per diem role delivering home health risk assessments. This position offers professionals the autonomy to craft their own schedules while making a significant impact on patient well-being through focused, one-on-one care.
Schedule:
- Flexible shifts available Monday through Sunday
- Option to create your own schedule
- Weekly or bi-weekly hourly commitment tailored to your preference
Key Responsibilities:
- Complete annual home health risk assessments for Medicare Advantage members
- Evaluate patients' medical history, current conditions, and medication usage
- Educate patients on medical conditions and prescribed medications
- Identify gaps in care, offering recommendations for optimal health outcomes
- Visits may include annual assessments or post-discharge check-ins
- No requirement for creating treatment plans or prescribing medications
Patient visits will be conducted in-home throughout Portage Countyincluding areas such as Kent, Ravenna, and Streetsboroand may be scheduled in any of these locations during a shift. Each risk assessment visit averages 45 minutes, providing ample opportunity to offer high-quality and individualized patient care.
Qualifications:
- Active Nurse Practitioner license in Ohio
- Board Certification through the AANP or ANCC
- Current BLS (CPR) certification
- MSN degree from an accredited nursing program
- Minimum of 6 months of nurse practitioner experience preferred
This opportunity includes compensation of $80-$90 per evaluation, along with a mileage stipend and administrative time.
