Make a meaningful impact on patient care in a flexible, independent role as a Nurse Practitioner specializing in home health risk assessments. As part of this per diem opportunity, you will serve adult Medicare Advantage members by conducting comprehensive health evaluations in various locations across Portage County, including Kent, Ravenna, and Streetsboro. Your assessments will empower patients and help ensure continuity of care, allowing you to play a key role in the health and wellbeing of the community.

Schedule:

• Flexible shifts available Monday through Sunday
• Set your own schedule
• Choose minimum weekly or bi-weekly hour commitments

Core Responsibilities:

• Conduct annual and post-discharge home health risk assessments
• Review prior and current medical history, with attention to chronic conditions and medication use
• Educate patients about disease management and safe medication practices
• Identify potential gaps in care and offer actionable recommendations
• No requirement for prescribing medications or creating treatment plans

Qualifications:

• Active Nurse Practitioner license in Ohio
• Board Certification via AANP or ANCC
• Current BLS/CPR certification
• Masters degree in nursing (MSN) from an accredited program
• At least 6 months of recent Nurse Practitioner experience is preferred
