Job Description

A per diem Nurse Practitioner opportunity is available to conduct home health risk assessments for Medicare Advantage plan members within Hamilton County, OH. This role involves visiting patients in their homes located in areas including Cincinnati, Forest Park, and Norwood. Each assessment typically lasts around 45 minutes and may involve annual health assessments or post-discharge visits. You will evaluate medical histories, provide education, and identify gaps in care, helping to improve patient outcomes without the responsibility of treatment plan creation or medication prescribing.

Key Responsibilities:

  • Perform comprehensive annual home health risk assessments for Medicare Advantage members
  • Review and assess prior and current medical history, medical conditions, and medication use
  • Educate patients on health conditions and medication management
  • Identify gaps in care and recommend resolutions
  • Maintain documentation and communicate findings per protocol

Qualifications and Experience:

  • Active Nurse Practitioner license in Ohio
  • Current Basic Life Support (BLS) certification
  • Board Certified through AANP or ANCC
  • Minimum of 6 months recent NP experience preferred
  • Master of Science in Nursing (MSN) from an accredited institution

Schedule:

  • Flexible shifts available Monday through Sunday
  • Create your own schedule with minimum weekly or bi-weekly hourly commitments
