Job Description

Experienced Nurse Practitioners are sought for flexible, per diem home health roles focused on risk assessments within the Columbiana County region, including Perry Township, Salem, and East Liverpool. This position caters to Medicare Advantage plan members and requires travel to patient residences for each assessment. Assessments last approximately 45 minutes and may vary from annual health reviews to post-discharge visits. The role offers outstanding autonomyset your own schedule with shifts available any day of the week and a minimum commitment that fits your individual needs.

Schedule:

  • Choice of flexible shifts MondaySunday
  • Create your own schedule to fit weekly or bi-weekly commitments

Key Responsibilities:

  • Complete annual and periodic home health risk assessments for Medicare Advantage members
  • Review and evaluate patients medical history, existing conditions, and medication regimens
  • Deliver patient education tailored to medical conditions and prescribed therapies
  • Identify potential care gaps and recommend resolutions to optimize health outcomes
  • No requirement to create treatment plans or prescribe medications

Qualifications:

  • Valid Nurse Practitioner license within the state of Ohio
  • MSN degree from an accredited nursing school
  • Current Board Certification (AANP or ANCC)
  • Active CPR (BLS) certification
  • At least 6 months of recent NP clinical experience preferred

This opportunity provides excellent compensation of $80$90 per evaluation, with additional mileage stipends and administrative time covered.
