Job Description

Make a meaningful impact in home health by conducting comprehensive risk assessments for Medicare Advantage plan members. This per diem opportunity offers Nurse Practitioners (NPs) the unique flexibility to design their own schedule, ensuring work-life balance while delivering critical healthcare services in the community.

Schedule:

  • Flexible shifts offered Monday through Sunday
  • Set your own hours each week or bi-weekly, based on your availability

Key Responsibilities:

  • Perform annual risk assessments in patients homes in various Brown County, OH communities such as Georgetown, Mount Orab, Ripley, and Pleasant Township
  • Conduct thorough evaluations by reviewing prior and current medical history, current health conditions, and medication usage during 45-minute visits
  • Deliver personalized education focused on medical conditions and proper medication management
  • Recognize and document care gaps, making useful recommendations to ensure patient well-being
  • Maintain a non-prescribing, non-treatment planning roleyour expertise is focused on evaluation and assessment

Qualifications:

  • Active and unrestricted Nurse Practitioner license in Ohio
  • Current CPR (BLS) certification
  • Board Certified by AANP or ANCC
  • Six months or more of NP experience preferred
  • Master of Science in Nursing degree from an accredited institution
