Job Description

Providing in-home health risk assessments offers vital support to Medicare Advantage plan members in Greenville and wider Darke County, OH. This per diem position is ideal for Nurse Practitioners seeking a flexible schedule and impactful patient interactions within local communities including Greenville, Versailles, and Arcanum.

Key Responsibilities:

  • Perform annual home health risk assessments averaging 45 minutes per visit
  • Assess patients medical and medication histories
  • Educate members on their medical conditions and medications
  • Identify gaps in care and suggest actionable recommendations
  • Conduct visits across Darke County; shift locations may vary
  • Note: No treatment plan creation or prescribing required

Qualifications:

  • Valid Nurse Practitioner License in Ohio
  • Current BLS (CPR) certification
  • Board Certification through AANP or ANCC
  • Preferred: Minimum 6 months of recent Nurse Practitioner experience
  • Master of Science in Nursing (MSN) from accredited institution

Schedule:

  • Create your own schedule with flexible shifts Monday through Sunday
  • Minimum weekly or bi-weekly hourly commitment as per your availability
