This vital role centers on providing comprehensive home health risk assessments for Medicare Advantage plan members across various locations in Fayette County, including Washington Court House, Jeffersonville, and Bloomingburg. Each home health visit typically lasts around 45 minutes, providing a valuable chance to make a difference in patients lives through thorough evaluation and education.

Schedule:

  • Flexible shifts available Monday through Sunday
  • Set your own schedule for maximum work-life balance
  • Meet a minimum weekly or bi-weekly hourly commitment

Key Responsibilities:

  • Perform annual or post-discharge health risk assessments in home settings
  • Review and document current and prior medical history, including medication usage
  • Educate patients on their conditions, treatment plans, and medications
  • Identify and address gaps in care, making recommendations for improved health outcomes
  • Focus exclusively on assessmentsno prescribing or treatment plan creation required

Qualifications:

  • Active Nurse Practitioner License in OH
  • Board Certification (AANP or ANCC)
  • Current Basic Life Support (CPR) certification
  • Completed MSN from an accredited nursing school
  • At least 6 months of experience as a Nurse Practitioner (preferred)
