Job Description

Provide expert home health risk assessments as a Nurse Practitioner serving Medicare Advantage plan members across Highland County, OH. This per diem role offers flexible scheduling options, allowing you to create your own shifts between Monday and Sunday with a minimum weekly or bi-weekly hourly commitment. You will perform thorough assessments that typically last about 45 minutes, focusing on annual health evaluations and post-discharge visits to ensure optimal patient care.

Key Responsibilities:

  • Conduct comprehensive annual home health risk assessments for Medicare Advantage members.
  • Review and evaluate prior and current medical history, ongoing conditions, and medication regimens.
  • Educate patients on their medical conditions and medication use to promote understanding and compliance.
  • Identify care gaps and recommend appropriate interventions or referrals.
  • Note: This position does not require development of treatment plans or prescribing medications.

Qualifications:

  • Valid Nurse Practitioner license in Ohio.
  • Current Basic Life Support (BLS) CPR certification.
  • Board Certification through AANP or ANCC.
  • Preferably at least 6 months of recent Nurse Practitioner experience.
  • Master of Science in Nursing (MSN) from an accredited institution.
