If youre an experienced Nurse Practitioner passionate about delivering quality care in a home health setting, consider this per diem opportunity conducting risk assessments throughout Preble County and its communities of Eaton, Camden, and Lewisburg. As part of this role, youll engage with Medicare Advantage members in their homes, ensuring comprehensive evaluations that support ongoing health and wellness.
Schedule:
- Flexible shifts available Monday through Sunday

Core Responsibilities:
- Complete in-home annual risk assessments for Medicare Advantage members
- Review prior and current medical histories, ongoing conditions, and medication regimens
- Educate patients on their diagnoses and medication management
- Identify care gaps and offer actionable recommendations to address them
- Visits may include annual assessments or post-discharge check-ins; treatment planning and prescribing are not required
Desired Qualifications:
- Nurse Practitioner (NP) License in OH
- Board Certification by AANP or ANCC
- Current CPR certification (BLS)
- MSN degree from an accredited nursing program
- At least six months of NP experience preferred
Compensation: Youll earn $80$90 per completed evaluation, in addition to a mileage stipend and paid administrative time, allowing you to maximize both your time and expertise on a flexible schedule.
