Job Description
This per diem Nurse Practitioner role offers the opportunity to conduct home health risk assessments for Medicare Advantage plan members throughout Ashtabula County, Ohio. The assessments typically take 45 minutes and may include annual health evaluations or post-discharge visits. This position allows providers to create their own schedules with flexible shifts available Monday through Sunday. Candidates must have a valid Ohio Nurse Practitioner license and relevant clinical experience.
Key Responsibilities:
- Conduct comprehensive annual home health risk assessments assessing medical history, current conditions, and medication usage.
- Educate patients on their medical conditions and medications to promote improved health outcomes.
- Identify care gaps and recommend appropriate resolutions to support optimal patient care.
- Perform assessments without the responsibility of creating treatment plans or prescribing medications.
Qualifications:
- Valid Nurse Practitioner license in Ohio.
- Current CPR certification (BLS).
- Board certification through AANP or ANCC.
- Minimum six months of current NP practice experience preferred.
- Master of Science in Nursing (MSN) from an accredited institution.
