Job Description

Deliver high-quality care as a Nurse Practitioner specializing in home health assessments on a per diem basis. This position empowers you to create your own schedule and serve Medicare Advantage plan members by conducting in-home risk assessments throughout Brown County, OH, including areas like Georgetown, Mount Orab, and Ripley. Shifts are flexiblework between Monday and Sunday with minimum weekly or bi-weekly hourly commitment options.

Key Responsibilities:

  • Perform comprehensive annual and post-discharge home health risk assessments for Medicare Advantage members
  • Review and assess prior and current medical history, active health conditions, and medication regimens
  • Educate patients about their medical conditions and appropriate medication usage
  • Identify care gaps and recommend strategies for improvement
  • Focus solely on evaluationno treatment planning or prescription authority required

Qualifications:

  • Active Nurse Practitioner license in Ohio
  • Current BLS (CPR) certification
  • National Board Certification through AANP or ANCC
  • Minimum of 6 months recent Nurse Practitioner experience (preferred)
  • Master of Science in Nursing (MSN) from an accredited nursing program
