Take the opportunity to make a real impact as a Nurse Practitioner, providing personalized care through home health risk assessments for Medicare Advantage plan members in Meigs County, OH. This per diem position offers flexibility with your schedule and the satisfaction of supporting patients in their homes across locations such as Salisbury Township, Pomeroy, Rutland Township, and Chester Township.
Schedule:
- Flexible shifts available Monday through Sunday
- Customize your own schedule
- Choose your preferred minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Perform comprehensive annual and post-discharge risk assessments for Medicare Advantage members in their homes
- Review and evaluate patients prior and current medical history, conditions, and medication history
- Educate patients about their health conditions and prescribed medications
- Identify any care gaps and make recommendations to support better health outcomes
- No requirement for creating treatment plans or prescribing medications
Qualifications:
- Active Nurse Practitioner License in Ohio
- Current BLS/CPR certification
- Board Certification through AANP or ANCC
- Minimum of six months of recent Nurse Practitioner experience preferred
- MSN degree from an accredited nursing program
