Key Responsibilities:
- Perform comprehensive home health risk assessments, typically lasting 45 minutes per visit
- Review and evaluate prior and current medical history, ongoing conditions, and medication regimens
- Deliver patient education on disease management and prescribed medications
- Identify care gaps and provide actionable recommendations for improved health outcomes
- Complete annual assessments and occasional post-discharge follow-ups
- Note: No diagnosis, treatment plans, or medication prescribing is required
Qualifications:
- Active Nurse Practitioner license in Ohio
- Board Certification through AANP or ANCC
- Valid Basic Life Support (BLS/CPR) certification
- Master of Science in Nursing (MSN) from an accredited program
- At least 6 months of recent NP experience is strongly preferred
