Seeking a seasoned Nurse Practitioner with experience in home health for a per diem role performing home health risk assessments throughout Portage County, OH. This position allows you to create your own flexible schedule, choosing shifts between Monday and Sunday and setting your minimum weekly or bi-weekly hourly commitment.
You will be conducting home visits for Medicare Advantage plan members living in Franklin Township and the greater Portage County area, possibly including locations such as Kent, Ravenna, and Streetsboro. Each risk assessment typically takes about 45 minutes and may involve annual assessments or post-discharge visits.
Key Responsibilities:
- Conduct comprehensive annual home health risk assessments for a Medicare Advantage health plan
- Review and document patients prior and current medical history, conditions, and medication use
- Educate patients about their health status, medical conditions, and prescribed medications
- Identify gaps in patient care and make recommendations to address these needs
- No creation of treatment plans or medication prescribing required
Schedule:
- Flexible shifts available any day of the week (Monday-Sunday)
- Set your own schedule based on availability
- Meet minimum weekly or bi-weekly hours as preferred
Qualifications:
- Active Nurse Practitioner license in Ohio
- Current CPR certification (BLS)
- Board certification through AANP or ANCC
- MSN degree from an accredited nursing program
- At least 6 months of recent Nurse Practitioner experience preferred
Compensation for this role is $80-$90 per evaluation, in addition to a mileage stipend and compensation for administrative time.
