Job Description
Deliver essential care directly to patients in their homes as a Nurse Practitioner specializing in home health assessments. This per diem opportunity offers flexibility and autonomy while making a positive impact on Medicare Advantage members across Portage County, including Franklin Township and the surrounding areas such as Kent, Ravenna, and Streetsboro.
Schedule:
- Flexible shifts available Monday through Sunday
- Select your own working days
- Meet a minimal weekly or bi-weekly commitment
Key Responsibilities:
- Perform thorough annual risk assessments for Medicare Advantage patients in their homes
- Review and evaluate patients current and past medical history, conditions, and medication use
- Educate patients and their caregivers about ongoing health conditions and appropriate medication utilization
- Identify any gaps in care delivery, providing actionable recommendations for follow-up
- Focus on assessment and guidanceno treatment planning or prescribing duties required
Qualifications:
- Active Nurse Practitioner license in Ohio
- Current CPR certification (BLS)
- Board Certification through AANP or ANCC
- Minimum of 6 months of current nurse practitioner experience preferred
- Master of Science in Nursing (MSN) from an accredited institution
This position offers compensation of $80-$90 per completed evaluation, in addition to a mileage stipend and pay for administrative time.
