An exciting per diem opportunity is available for an experienced Nurse Practitioner to deliver home health risk assessments for Medicare Advantage plan members in Fayette County, OH. This role allows you to provide meaningful care while enjoying flexible scheduling and autonomy. Assignments may be in areas such as Washington Court House, Jeffersonville, and Bloomingburg during any given shift.
Schedule:
- Flexible shifts available Monday through Sunday
- Create your own work schedule
- Minimum weekly or bi-weekly hourly commitment
Key Responsibilities:
- Perform in-home health risk assessments averaging 45 minutes per visit
- Assess patients medical history, current conditions, and medication history
- Conduct annual health evaluations and post-discharge visits as needed
- Educate patients about their diagnoses and medications
- Identify gaps in care and make recommendations for follow-up
- No requirement for prescribing medications or developing treatment plans
Qualifications:
- Must hold a Nurse Practitioner License in Ohio
- Active board certification with AANP or ANCC
- Completion of MSN from an accredited nursing school
- Valid CPR (BLS) certification
- Minimum six months of experience as a Nurse Practitioner is preferred
Compensation includes $80$90 for each evaluation completed, along with a mileage stipend and paid administrative time.
