This position centers around conducting thorough, 45-minute Medicare Advantage evaluations, which may include annual health review visits or post-discharge assessments. Youll visit members in locations such as Washington Court House, Jeffersonville, and Bloomingburg, ensuring seamless care transitions and health monitoring.

Schedule:

• Flexible shifts available Monday through Sunday
• Set your own hours to match your lifestyle
• Commit to a minimum weekly or bi-weekly hourly schedule

Responsibilities:

• Complete annual health risk assessments for Medicare Advantage plan members within home environments.
• Review and evaluate individuals past and current medical history, existing conditions, and medication lists.
• Educate plan members about their health conditions and medication regimens.
• Detect care gaps and share recommendations to help resolve them.
• No responsibility for creating treatment plans or prescribing medications.

Qualifications:

• Active Nurse Practitioner license in Ohio
• Current CPR certification (BLS required)
• Board Certification (AANP or ANCC)
• MSN degree from an accredited institution
• Minimum of 6 months recent experience as a practicing NP preferred

Nurse Practitioners in this role earn $80$90 for each completed evaluation. Additional compensation includes a mileage stipend for travel between visits and payment for administrative time.
