This per diem position is ideal for those seeking flexibility and variety while interacting with Medicare Advantage members across Miami County, including Troy, Piqua, and Tipp City.
Schedule:
- Flexible shifts available any day from Monday through Sunday
- Self-schedulingset your own hours each week
- Minimum weekly or bi-weekly hourly commitment required
Key Responsibilities:
- Complete annual home health risk assessments for Medicare Advantage plan members during home visits (approximate duration: 45 minutes per evaluation)
- Perform comprehensive evaluations of medical history, current conditions, and medication usage
- Educate members on their medical conditions and prescribed medications
- Identify gaps in care and offer recommendations for improvement
- No treatment planning or medication prescribing involved
Qualifications:
- Active Nurse Practitioner license in Ohio
- Current Basic Life Support (BLS) CPR certification
- National Board Certification through AANP or ANCC
- Minimum of 6 months of recent NP experience is preferred
- Master of Science in Nursing (MSN) from an accredited program
