Make a meaningful impact on patient care by conducting home visits and health risk assessments for Medicare Advantage plan members throughout Miami County. This per diem opportunity allows for flexibility and autonomy, with nurse practitioners evaluating and supporting members in Troy, Piqua, Tipp City, and nearby communities. Each home health risk assessment averages 45 minutes, covering annual health evaluations or post-discharge check-ins.

Schedule:

• Flexible scheduling available Monday through Sunday
• Self-selected shiftscreate a schedule that works for you
• Minimum weekly or bi-weekly hours as preferred

Key Responsibilities:

• Complete annual home health risk assessments for Medicare Advantage plan members
• Review and evaluate patients prior and current medical history, conditions, and medication use
• Educate members on medical conditions and medication adherence
• Identify gaps in care and make appropriate recommendations
• Focus solely on assessmentno prescribing or treatment planning required

Qualifications:

• Active Nurse Practitioner license in Ohio
• MSN degree from an accredited nursing program
• Current BLS/CPR certification
• Board Certification through AANP or ANCC
• At least 6 months of experience as a nurse practitioner strongly preferred

This role provides nurse practitioners with $80$90 per evaluation, alongside a mileage stipend and compensation for administrative time.
