Job Description

Providing comprehensive healthcare to residents in nursing home and assisted living settings, this role emphasizes in-place treatment to reduce unnecessary hospital visits and support successful transitions to home care. The position involves monthly, and as needed, health assessments to enhance patient outcomes and avoid emergency interventions.

Schedule

  • Monday to Friday
  • 8 AM to 5 PM
  • No weekends or holidays

Responsibilities

  • Develop individualized problem lists and medical treatment plans for residents
  • Deliver primary, preventative, urgent, and acute care onsite
  • Identify and stratify high-risk patients to schedule more frequent visits
  • Formulate contingency plans for complex or high-risk conditions
  • Collaborate closely with residents primary care physicians and facility staff, including Directors of Nursing
  • Conduct hands-on bedside care, develop care plans, and place orders in coordination with the care team
  • Perform assessments for patients transitioning from hospital to facility, ensuring safe discharge plans
  • Manage a caseload of members across multiple nursing facilities, generally visiting one facility per day with potential for additional visits to other locations for acute patient assessments

Qualifications and Experience

  • Master of Science in Nursing (MSN) degree
  • Current Oregon Nurse Practitioner license
  • Basic Life Support (BLS) certification
