This per diem position offers the chance to make a meaningful difference by traveling throughout Preble County and performing comprehensive risk assessments with Medicare Advantage members in their homes. Schedule:
- Flexible shifts available Monday through Sunday
- Design your own work schedule
- Choose a minimum weekly or bi-weekly hourly commitment that suits your lifestyle
Key Responsibilities:
- Conduct comprehensive annual home health risk assessments in members residences
- Travel to a variety of locations including Eaton, Camden, and Lewisburg within Preble County
- Review and document past and current medical histories, medical conditions, and medication usage
- Educate patients about their medical conditions and prescribed treatments to promote better health outcomes
- Identify any gaps in care and offer clear recommendations to optimize patient wellness
- Focus exclusively on assessment; prescribing medications or developing treatment plans is not required
Qualifications:
- Active Nurse Practitioner License in Ohio
- Current BLS (CPR) certification
- Board certification through AANP or ANCC
- At least six months of recent Nurse Practitioner experience preferred
- MSN from an accredited nursing program
