This per diem position offers the chance to make a meaningful difference by traveling throughout Preble County and performing comprehensive risk assessments with Medicare Advantage members in their homes. Schedule:

  • Flexible shifts available Monday through Sunday
  • Design your own work schedule
  • Choose a minimum weekly or bi-weekly hourly commitment that suits your lifestyle

Key Responsibilities:

  • Conduct comprehensive annual home health risk assessments in members residences
  • Travel to a variety of locations including Eaton, Camden, and Lewisburg within Preble County
  • Review and document past and current medical histories, medical conditions, and medication usage
  • Educate patients about their medical conditions and prescribed treatments to promote better health outcomes
  • Identify any gaps in care and offer clear recommendations to optimize patient wellness
  • Focus exclusively on assessment; prescribing medications or developing treatment plans is not required

Qualifications:

  • Active Nurse Practitioner License in Ohio
  • Current BLS (CPR) certification
  • Board certification through AANP or ANCC
  • At least six months of recent Nurse Practitioner experience preferred
  • MSN from an accredited nursing program
