Job Description

Experienced Nurse Practitioners with a passion for community-based care are invited to bring their expertise to an engaging home health role. This per diem position allows for flexible scheduling, making it ideal for clinicians seeking autonomy and work-life balance.

Schedule:

  • Flexible shifts available Monday through Sunday.
  • Create your own schedule to accommodate your availability.
  • Minimum weekly or bi-weekly hourly commitment.

Key Responsibilities:

  • Perform comprehensive home health risk assessments for Medicare Advantage plan members.
  • Collect and evaluate detailed medical histories, current conditions, and medication histories.
  • Deliver clear education to members regarding their medical conditions and medications.
  • Identify care gaps and share recommendations for improvement and follow-up.
  • Visits may include annual health assessments and post-discharge evaluations.
  • The focus is on assessment only; no need to create treatment plans or prescribe medications.

Qualifications:

  • Active Nurse Practitioner license in Ohio.
  • Board Certification through AANP or ANCC.
  • MSN degree from an accredited nursing program.
  • Current CPR (BLS) certification.
  • At least 6 months of recent experience as a Nurse Practitioner is preferred.
