Nurse Practitioners are needed to perform home health risk assessments for Medicare Advantage plan members living in Cuyahoga County, OH. This role involves visiting patients in their homes across various cities including Parma, Strongsville, Lakewood, and Cleveland. Each assessment typically takes about 45 minutes and may involve annual health evaluations or post-discharge visits. The position offers flexible, per diem scheduling with a minimum commitment of weekly or bi-weekly hours.
Key Responsibilities:
- Conduct comprehensive annual home health risk assessments on Medicare Advantage members.
- Review and evaluate patients medical history, current conditions, and medication usage.
- Provide education and support related to medical conditions and prescribed medications.
- Identify gaps in care and offer recommendations to improve patient outcomes.
- Note: This role does not require creating treatment plans or prescribing medications.
Qualifications:
- Active Nurse Practitioner license in Ohio.
- Certification in Basic Life Support (BLS).
- Board Certification through AANP or ANCC.
- Preferred: Minimum of 6 months current NP experience.
- Master of Science in Nursing (MSN) from an accredited institution.
Schedule:
- Flexible shifts available Monday through Sunday.
- Ability to create your own schedule based on availability.
- Minimum commitment of weekly or bi-weekly hours.
