Take the opportunity to bring your expertise to home health nursing as a Nurse Practitioner, conducting risk assessments for Medicare Advantage plan members within Preble County. In this per diem role, you will deliver meaningful care in patients homes, helping to identify health needs and bridge gaps in careall while enjoying the flexibility of managing your own schedule.
Schedule:
- Flexible shifts available seven days a week
- Choose your own work hours
- Minimal hourly commitment required on a weekly or bi-weekly basis
Key Responsibilities:
- Perform comprehensive annual or post-discharge risk assessments in home settings
- Evaluate medical histories, current conditions, and medication regimens
- Educate patients about their health conditions and medications
- Spot gaps in care and recommend actionable solutions
- No treatment planning or prescribing necessary
Qualifications:
- Licensed Nurse Practitioner in Ohio
- Current CPR (BLS) certification
- Board certification via AANP or ANCC
- MSN degree from an accredited nursing school
- At least 6 months of recent NP experience preferred
As part of this fulfilling opportunity, youll receive compensation of $80$90 per evaluation, with additional mileage stipend and administrative time provided.
