Job Description

Make an impact on community health by performing risk assessments for Medicare Advantage members as a Nurse PractitionerHome Health. In this per diem role, youll use your expertise to conduct comprehensive assessments in patients homes throughout Preble County, including areas such as Eaton, Camden, and Lewisburg. This is an opportunity for dedicated NPs to enjoy both flexibility and autonomy while providing critical healthcare services.

Schedule & Flexibility:

  • Flexible shifts available Monday through Sunday
  • Create your own schedule for work-life balance
  • Minimum weekly or bi-weekly hourly commitment required

Key Responsibilities:

  • Conduct annual home health risk assessments for Medicare Advantage health plan members
  • Evaluate patients prior/current medical histories, including existing medical conditions and medication usage
  • Deliver patient education on chronic conditions and medication management
  • Identify care gaps and provide actionable recommendations for follow-up, without the need for creating treatment plans or prescribing medications
  • Average visit duration is approximately 45 minutes

Qualifications & Experience:

  • Nurse Practitioner (NP) License in Ohio is required
  • Board Certification through AANP or ANCC
  • Current CPR certification (BLS)
  • MSN degree from an accredited nursing school
  • Minimum of 6 months of current NP experience preferred
